Welcome to the home of WNY IPA,
the Western New York Long-Term Care Network Independent Provider Association.

Our organization is the first of its kind in this region to focus on expanding long-term health care options for patients and providers through a unique, aligned approach to care.

WNY IPA is comprised of 15 health care facilities in Erie, Niagara and Chautauqua Counties that provide an extensive breadth of services including: skilled nursing care, subacute & outpatient rehabilitation, adult day care, assisted living, retirement living and home care. Our collaboration is built on the foundation of maximizing outcomes and efficiency, while offering unparalleled services to thousands of residents and patients each year.

WNY IPA's core objectives focus on:

  • Understanding the needs of the community as they relate to varying levels of care for the elderly, including those who are especially frail

  • Partnering to provide efficiencies in operations and cost containment

  • Early detection and proactive treating of residents/patients before declines occur

  • Enhancing care and improving patient experiences
  • Developing and implementing clinically proven best practice programs and protocols that exceed national and state averages for quality care

  • Developing sophisticated clinical programs to ensure compliance to state regulations and federal mandates

  • Avoiding unnecessary hospital stays

  • Offering direct admissions to bypass acute hospital stays, depending on clinical need
  • Collaborating rather than competing on services and programs that will benefit the patients we serve and improve their overall quality of life

  • Bridging the gap between hospital and home. Our short-term In-patient postacute and outpatient rehabilitation programs provide smooth transitions and minimize your likelihood for potential re-hospitalizations

WNY IPA believes that advancing the quality of the health care delivery system has a direct impact on the health and well-being of the individuals entrusted to our care, as well as our region as a whole. Our member organizations are recognized for their legacies of care, best practice reform and outstanding ratings from the federal and state governments.
